29 CFR 1926.451(b)(1): Each platform on all working levels of the scaffold was not fully planked or decked between the front uprights and the guardrail supports. At the north side of the building, the employer did not ensure that employees performing effis installation operations from the tubular welded frame scaffold were protected from a fall hazard of approximately fifteen (15) feet in that where it takes three planks to fully plank the working surface there were only two installed.
