29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around that hole:  a. On or about March 29, 2018, employees tearing off an old roof system on a 4:12 roof were not protected from stepping into or thru a 2 x 2 skylight, exposing the employees to fall hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):   a. On or about March 29, 2018, employees tearing off an old roof system on a 4:12 pitch roof were not protected from falling 8.5 feet to the ground by the use of a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a. On or about March 29, 2018, front of home, the ladder used to access and exit from the roof was not extended 3 feet above the landing, exposing employees to fall hazards.
