29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:  On or about March 27, the employer had not made an evaluation to determine if the stilling wells at the gaging stations serviced by the Escanaba Field Office were permit required confined spaces. The stilling wells contained water engulfment hazards and potentially hazardous atmospheres.

29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  On or about March 28, 2018, the employer did not ensure danger signs were posted at the Ford River near Hyde gaging station with had a stilling well  where employees could enter through a floor hatch. Entry into the stilling well could expose employees to water engulfment hazards and potentially hazardous atmospheres.   NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.
