TM Holdings d.b.a. Midtown Dry Cleaners located at 99 B Whittlesey Avenue in Norwalk, Ohio; On or about April 3, 2018 the employer did not develop and implement the elements of a written hazard communication program. Employees were potentially exposed to hazards associated with PERC including, but not limited to, malaise, dizziness, headache, and fatigue.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical: a) M.K. TM Holdings d.b.a. Midtown Dry Cleaners located at 99 B Whittlesey Avenue in Norwalk, Ohio; On or about April 3, 2018 the employer did not maintain the most recent version of the safety data sheet corresponding to the Dowper Solvent Perchloroethylene, in that the SDS on file was dated 1997. Employees were potentially exposed to hazards associated with PERC including, but not limited to, malaise, dizziness, headache, and fatigue.
