29 CFR 1910.147(e)(2)(i): The work area shall be checked to ensure that all employees have been safely positioned or removed. On 3/28/2018 an employee was diagnosing why the nut feeder for a spot welder was not properly working. During this process the employee engaged the wrong air valve engaging the drawer this resulted in the right hand of the employee being pinched between the metal drawer guide and the metal drawer resulting in an amputation.

29 CFR 1910.212(a)(2): General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. On or about 3/28/18 an employee was working on the air valves of a spot welder. During this process the employee engaged the air valve for the drawer resulting in closing the drawer trapping his right hand between the drawer and the guard.
