29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment were not independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: a) Roof: On or about April 3, 2018 and at times prior there to, the employer allowed two roofers to attach their respective lifelines to the same anchor device which was not designed for that type of use and exposed the employees to injuries associated with falls up to 9-feet to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Roof: On or about April 3, 2018 and at times prior there to, the employer did not ensure fall protection was used while conducting shingle installation related activities which exposed an employee to injuries associated with falls up to 9-feet to the ground below. Luis Bueno Martinez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1007288 citation number 1 item number 1 and was affirmed as a final order on December 16, 2014, with respect to a workplace located at 24536 Huxford Street, Daphne, Alabama 36526.
