29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour: a) Mor-Storage, Inc. at 3010 I-70 Business Loop, Grand Junction, CO 81501: On and before April 3, 2018, the employer did not ensure that oxygen cylinders in storage were separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet or by a noncombustible barrier at least 5 feet high having a fire-resistance rating of at least one-half hour. The employer stored cylinders of oxygen and oxyacetylene together, located in an area where other combustible materials were also stored within 20 feet. This condition exposed employees to burn hazards.

29 CFR 1904.41(a)(2): The employer did not electronically submit information from the OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee if the establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and this establishment is classified in an industry listed in appendix A to subpart E of this part. This information was not submitted once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form. (a) Mor-Storage, at 3010 I-70 Business Loop, Grand Junction, CO 81501; On and before April 3, 2018, the employer was in the business of manufacturing portable storage shed, and operated with 33 employees. The employer's 206 log reflected two injuries involving days away from work, and two injuries involving job transfer or restrictions. The employer did not electronically submit information from the OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee. This condition resulted in a failure to report data to OSHA.
