29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6):  a) Lockout Tagout Procedure Document EHS-22. - On or about April 3, 2018, the procedure for general sequence was not as required by 29 CFR 1910.147(d)(1) through (d)(6) in that stored energy was identified to be dissipated in paragraph 7.2.3 before lockout devices were to be applied in paragraph 7.2.4.  b) Lockout Tagout Procedure Document RSOP-103. - On or about April 3, 2018, the procedure for general sequence was not as required by 29 CFR 1910.147(d)(1) through (d)(6) in that stored energy was identified to be dissipated in Step 3 before lockout devices were to be applied in Step 4.
