29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:      a)  Spec Fab near JC5, on or about 4/3/18: DeWalt DW756 pedestal grinder, right side adjustable tongue guard at the top measured 1-1/8 inches from the abrasive wheel.    b)  Spec Fab near JC5, on or about 4/3/18:  DeWalt DW756 pedestal grinder, left side adjustable tongue guard at the top measured 5/8 inches from the abrasive wheel.    c)  B3 Heavy Fab, on or about 4/3/18:  Craftsman pedestal grinder, left side adjustable tongue guard at the top measured 3/4 inches from the abrasive wheel.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a)  B3 Heavy Fab, on or about 4/3/18:  Spartan Ironworker, #1W11OD, used to notch angle iron, was not guarded to prevent point of operation hazards.    b)  B3 Heavy Fab, on or about 4/3/18:  Milwaukee DR94 magnetic drill was not equipped with a guard to prevent contact with the rotating chuck and tool.     c)  B3 Heavy Fab, on or about 4/3/18:  Jancy Engineering DR96 magnetic drill was not equipped with a guard to prevent contact with the rotating chuck and tool.    d)  B3 Heavy Fab, on or about 4/3/18:  Jancy Engineering Slugger magnetic drill was not equipped with a guard to prevent contact with the rotating chuck and tool.      The Cameron Manufacturing & Design, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA inspection number 1123851, citation number 1, item number 5 and was affirmed as a final order on 6/23/17, with respect to a workplace located at 727 Blostein Boulevard, Horseheads, NY 14845.

29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator and at least annually thereafter:  a)  Cameron Manufacturing & Design, Inc., on or about 4/3/18:   Employees who are required to wear half-mask respirators when spray painting were not fit tested on an annual basis.  Fit tests were last provided in February and July of year 2016.   Cameron Manufacturing & Design, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(f)(2) which was contained in OSHA inspection number 1123851, citation number 1, item number 2b, and was affirmed as a final order on 6/23/16, with respect to a workplace located at 727 Blostein Boulevard, Horseheads, NY 14845.
