29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: a) Steam boiler addition - On or about March 29, 2018 and at times prior thereto, the employer did not provide applicable training to employees performing construction activities including but not limited to; steel erection, rigging, and working with cranes. Employees were constructing a systems-engineered building. On March 29, 2018 an employee of Hopkins Welding & Crane was fatally injured due to a partial collapse of the building.

29 CFR 1926.754(a): Structural stability was not maintained at all times during the erection process: a) Steam boiler addition - On or about March 29, 2018 and at times prior thereto, employees were exposed to an unstable systems-engineered metal building as the columns did not have any anchors in them. On March 29, 2018 an employee of Hopkins Welding & Crane was fatally injured due to a partial collapse of the building.
