29 CFR 1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer is failing to protect employees from amputation hazards associated with using inadequately guarded shop equipment.  The most recent occurrence of this was evident at the facility located at 7409 Irvington, Road, Omaha, NE where the employer has employees using the equipment in the shop to repair and create test equipment without having guarded against the amputation hazards.  Instances include but are not limited to:   i.     Employees using the Manhattan MSC Heavy Duty Table Top Drill Press; model 951220, MFG number 238836  used in the fabrication of parts or repair of equipment without having a guard over the chuck.   ii.    Employees using the MSC Milling and Drilling Machine model number 54162761, serial number 035102 used in the fabrication of parts or repair of equipment without having a guard over the chuck.   iii.   Employees using the ENCO Lathe, model number 110-1351; used in the fabrication of parts or repair of equipment without having a guard over the rotating drum.
