29 CFR 1915.152(b)(4): The employer failed to document an occupational hazard assessment which should have included: Occupations involved in the survey, the date in which the survey took place and the name of the person who performed the hazard assessment. a) Date of Violation: On or about 04-04-2018, located at Colonna's Shipyard, Inc., 400 East Indian River Road. Norfolk, VA 23523, located onboard Military Sealift Command vessel USNS Zeus, employee(s) were exposed to hazards relevant to ship repair without an occupational hazard assessment identifying what shipboard hazards the security guards were exposed to during the course of their duties. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employee(s) and their representatives were informed of this abatement.
