29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: (a) On or about 04/04/2018 - Engine Room, the employer failed to document and ensure compliance with recognized and generally accepted good engineering practices (RAGAGEP) to protect employees exposed to ammonia hazards as specified in the employer's chosen RAGAGEP, International Institute of Ammonia Refrigeration (IIAR) ANSI/IIAR 2-2008a, section 13.1.10.2 in that the door from the Engine room to the Offal room was not equipped with panic-type hardware.
