29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a)  Mezzanine Administrative Storage Area;  Employees did not have fall protection from an unguarded edge (with an approximate 15 foot fall) when the roll up door was raised.

29 CFR 1910.28(b)(3)(iv): The employer did not ensure each employee was protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used:  a)  Mezzanine;  A platform above a fixed ladder did not have a self-closing gate at the ladder opening. This exposes employees to fall hazards.
