29 CFR 1926.451(c)(2)(v): The employer used forklifts to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied: a) Delmar Public Library - On or about November 6, 2017, the employer lifted employees to heights of approximately 20 feet above grade while standing on a temporary work platform (a stack of sheathing materials) which was not secured to the forks of a (JLG) Skytrak 6042 Telehandler.
