29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening was less than 39 inches (1.0 m) above the walking/working surface, was not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system: On or about April 5, 2018, employees were exposed to a fall hazard of approximately 30 feet while unloading plumbing fixtures next to a wall opening and fall protection was not used.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. On or about April 5, 2018, employees were exposed to a fall hazard up to 30 feet while unloading plumbing fixtures through a wall opening and the employer did not ensure the employees received training by a competent person to identify the control method to be used.
