The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person (s) performing the training or evaluation: (a) This violation occurred on or about April 4, 2018, at a job site located at 2955 Blackhorn Street, first floor parking garage at the Ardent Tower West Village Block 7B, where an employee was exposed to struck-by hazards while walking adjacent to a Skytrak JLG 8042 forklift and holding on to scaffold parts that were on the forks; (b) This violation occurred on or about April 4, 2018, at a job site located at 2955 Blackhorn Street, first floor parking garage at the Ardent Tower West Village Block 7B, where employees operating a Skytrak JLG 8042 forklift were not trained and certified.
