Employees engaged in roofing operations were exposed to, but not limited to, fall hazards, struck-by hazards, and other hazards associated with roofing operations in that no program existed or was maintained as to instruct employees on the safe work practices at the jobsite located at 8427 W Meadow Pass Wichita KS.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  Employees engaged in jobsite cleanup, working on the ground under roofing operations, at the jobsite located at 8427 W Meadow Pass Wichita KS, were exposed to struck-by hazards in that no head protection was provided and/or utilized where roofing materials were being tossed off the roof directly above the employee.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  Employees engaged in access and egress from the roof located at the jobsite at 8427 W Meadow Pass were exposed to fall hazards of approximately 11 feet in that the ladder utilized did not extend 3 feet above the eve of the roof.
