29 CFR 1904.39(a)(2): Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, the employer did not report the in-patient hospitalization, amputation, or loss of an eye to OSHA: (a) Evolve Logistics Group, 510 Industrial Drive, Lewisberry Pennsylvania - An employee was struck by a 53 foot tractor trailer backing up and suffered broken clavicles, resulting in an employee hospitalization, on February 8, 2018. The employer failed to report this work-related hospitalization to OSHA within 24 hours. (The employer reported this hospitalization to OSHA on February 12, 2018.) Corrected during the inspection.
