29 CFR 1910.147(c)(1): The employer did not establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative. a. On or about March 17, 2018, employees were, servicing and maintaining the Jac Dough Divider machine and other machinery, and the company did not develop and implement an energy control program, thereby exposing employees to amputations.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. On or about March 17, 2018, an employee was injured while working which resulted in an amputation and the employer failed to report amputation within the 24 hours to OSHA.
