Employees were installing exterior light fixtures. Employees were lifted in a Bobcat T595 Loader with a shopmade work platform. The Bobcat was not specifically designed by the manufacturer to support scaffold platforms. The shopmade work platform was not designed and approved by the manufacturer for this task.
