29 CFR 1910.178(l)(3): Training program content. Powered industrial truck operators shall receive initial training in the following topics, except in topics which the employer can demonstrate are not applicable to safe operation of the truck in the employer's workplace. 29 CFR 1910.178(l)(3)(i)(G): Fork and attachment adaptation, operation, and use limitations; On and about April 5, 2018, employees were exposed to struck by hazards while operating forklifts when each operator had not been trained in the proper use of attachments in that an operator lifted a suspended load without using an approved lifting device.

29 CFR 1910.178(o)(1): Only stable or safely arranged loads shall be handled. Caution shall be exercised when handling off-center loads which cannot be centered. On and about April 5, 2018, employees were exposed to struck by and crushing hazards while lifting a truck bed frame with a Toyota forklift using a sling slid over a tine of the forklift.
