29 CFR 1926.602(c)(1)(ii): Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: Location: site On or about 4/4/18, an employee was working in a shop made platform which was mounted on an all-terrain forklift truck elevated to various heights while installing fascia and trim boards. The employer did not have a written approval from the forklift truck manufacturer to allow such addition/modification to the forklift.
