(b) New residential construction located on Lot #109, Second floor top plate: On or about 04/06/2018, and at times prior, the employer exposed employees to fall hazards of 19-feet, 8-inches to the lower level, in that employees were not provided with a fall protection system while installing plywood sheathing.
