29 CFR 1926.304(d): Portable, power-driven circular saw(s) were not equipped with a guard above and below the base plate or shoe:  (a) New residential construction located on Lot D-102, Work bench on back side: On or about 04/06/2018, and at times prior, the employer exposed employees to amputation hazards while using Makita Circular Saws, in that wood shims were used to keep the lower guards from retracting after the cuts were made through the siding materials. The lower guards remained in a fixed position and were not operable.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates, mud sills or other adequate firm foundation:  (b) New residential construction located on Lot D-102, Back side: On or about 04/06/2018, and at times prior, the employer exposed employees to fall/collapse hazards of up to 12-feet to the lower level while working from a pump jack scaffold system, in that supporting poles were not erected on a firm foundation as no base plates or mud sills were provided.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  (a) New residential construction located on Lot D-102, Back side: The employer exposed employees to fall hazards of 10-feet, 10-inches to 12-feet to the lower level, in that a fall protection system was not provided to employees installing siding from a pump jack scaffold system.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a) New residential construction located on Lot D-102, Cut bench: On or about 04/06/2018, and at times prior, the employer exposed employees to electrical shock hazards while using extension cords to power equipment, in that strain relief on one of the cords was damaged and not removed from service or repaired before the work shift began. No inspection of the electrical equipment was performed.
