29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of aerial lift being used and to understand the procedures to control or minimize those hazards: On or about April 6th, 2018, at the above jobsite, the employer did not provide hazard recognition training to an employee who was exposed to approximately 20-feet fall hazard, while working form the bucket of an aerial lift without the means of fall protection.
