29 CFR 1926.251(c)(4)(iii): Eyes in wire rope bridles, slings or bull wires were formed by wire rope clips or knots:     a)  Basement, Area Way - On or about April 5, 2018, workers were exposed to a struck-by hazard when a 500 lbs. portable conveyor system was suspended aloft by means of rigging where a 3/8 inch wire rope was formed into an eye by using wire rope clamps.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet), in addition, documentation demonstrating the abatement is complete must be included with your certification. This documentation may include, but is not limited to evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) was/were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:       a)  Basement, Area Way - On or about March 15, 2018, workers were removing a portion of a concrete beam while occupying a 14 feet deep excavation comprised of Class B soil, having vertical walls on two sides, and a third wall which was not adequately sloped, with no protective systems in place.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
