29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about April 10, 2018, during the removal of roofing material on a low slope (approximately 2/12) roof of a two story funeral home building, located at 271 North Park Avenue, Warren, Ohio, the employer did not ensure employees were protected from a fall of approximately 25 feet (roof to ground) by utilizing a guardrail system, fall protection system, and/or combination of warning line system and safety monitor system.

On or about April 10, 2018, during the installation of roofing material on a steeply pitched (approximately 6/12) roof of a two story funeral home storage garage building, located at 271 North Park Avenue, Warren, Ohio, the employer did not ensure an employee was protected from a fall of approximately 21 feet (roof to ground) by utilizing a guardrail system, safety net system, and/or personal fall arrest system.
