29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces. Location: Jobsite Employees were exposed to fall hazards while working, using aluminum ladders on slippery surface where ladders had not been secured. On or about April 10th 2018 an employee was injured from a fall while working on a ladder where the ladder was not secured on a slippery surface.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Location: Jobsite Employees were exposed to falls hazards while working on aluminum Little Giant ladders where each employee had not been trained by the employer in the safe use of said ladders.
