29 CFR 1910.176(a): Use of mechanical equipment. Where mechanical handling equipment is used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made. Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked. (a) The employer did not ensure that permanent aisles and passageways were appropriately marked. Exposing employees to struck-by hazards from mechanical handling equipment and vehicular traffic.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Following the work-related hospitalization of an employee that occurred on April 2, 2018, at a jobsite located at 921 W. Bethel Road, Bldg. 300, Suite 315, dock door 7, Coppell, Texas, the employer did not make an oral report to OSHA within 24 hours after the occurrence.
