29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On April 10, 2018 one employee was observed shingling the roof of a multi-story house being built at 6304 Dynamite Drive, McKinney, TX approximately nineteen (19) feet or more above the ground level below and was not protected from a fall hazard by using either guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On April 10, 2018 a thirty two (32) foot Werner extension ladder being utilized by employees to roof a two story home being built at 6304 Dynamite Dr., McKinney, TX was not placed with three (3) feet of its side rails above the upper landing exposing the employees using it to a fall hazard. b) On April 10, 2018 a twenty eight (28) foot Werner extension ladder (which had been separated into 2 separate ladders and the bottom half had been placed at the back of the house from the ground to the first floor roofs edge) being utilized by employees to roof a two story home being built at 6304 Dynamite Dr., McKinney, TX was not placed with three (3) feet of its side rails above the upper landing exposing the employees using it to a fall hazard. c) On April 10, 2018 a twenty eight (28) foot Werner extension ladder (which had been separated into 2 separate ladders and the top half had been placed at the back of the house from the first floors roof to the second floor roofs edge) being utilized by employees to roof a two story home being built at 6304 Dynamite Dr., McKinney, TX was not placed with three (3) feet of its side rails above the upper landing exposing the employees using it to a fall hazard.
