29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:    a) On April 10, 2018 a thirty two (32) foot Werner extension ladder being utilized by employees to roof a two story home being built at 6304 Dynamite Dr., McKinney, TX was not placed with three (3) feet of its side rails above the upper landing exposing the employees using it to a fall hazard.    b) On April 10, 2018 a twenty eight (28) foot Werner extension ladder (which had been separated into 2 separate ladders and the bottom half had been placed at the back of the house from the ground to the first floor roofs edge) being utilized by employees to roof a two story home being built at 6304 Dynamite Dr., McKinney, TX was not placed with three (3) feet of its side rails above the upper landing exposing the employees using it to a fall hazard.    c) On April 10, 2018 a twenty eight (28) foot Werner extension ladder (which had been separated into 2 separate ladders and the top half had been placed at the back of the house from the first floors roof to the second floor roofs edge) being utilized by employees to roof a two story home being built at 6304 Dynamite Dr., McKinney, TX was not placed with three (3) feet of its side rails above the upper landing exposing the employees using it to a fall hazard.
