Sheridan and E. 121st Street in Bixby, OK, employees were allowed to work in a trench more than 4 feet deep that had no ladder or other safe means access/egress within 25 feet. The employees were exposed to crushed-by and struck-by hazards.

The employees were exposed to the hazard of being crushed by or struck-by falling soil.
