29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilivered end: (a) New residential construction located at 2150 Canton View, Alpharetta, GA, Front side: On or about 04/10/2018, and at times prior, the employer exposed employees to collapse/fall hazards of 18-feet to the lower level, in that the cantilevered portion of the platform extended approximately 8-feet past the supporting ladder jack without being secured or barricaded as employees were engaged in cornice installation work.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: (a) New residential construction located at 2150 Canton View, Alpharetta, GA, Front side: On or about 04/10/2018, and at times prior the employer exposed employees to fall hazards of 18-feet to the lower level, in that a fall protection system was not provided to employees engaged in cornice board installation on a ladder jack scaffold system.
