29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:  Employees engaged in working in the kitchen area of the restaurant were exposed to an impact / tripping hazard due to the discharge sheet metal on the flame broiler not being able to be secured to the equipment.  The discharge sheet metal would fall off the flame broiler at any time either hitting the employees or creating a tripping hazard on the floor as employees walk pass the flame broiler.

29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards were present, or were likely to be present, which necessitated the use of personal protective equipment:  Employee(s) engaged in lighting the flame broiler manually at 1100 North 3rd Street, Lawrence, Kansas were exposed to burn hazards due to no written personal protective equipment hazard assessment being conducted to determine the type of personal protective equipment required when lighting the flame broiler manually.  The sheet metal surrounding the ignition area of the flame broiler was removed, then an employee(s) would use a lighter or place a burning paper towel inside the flame broiler to ignite the natural gas.
