29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. On April 10, 2018, located on the North side of the commercial roof structure, employees were exposed to a fall hazard while conducting tear off activities from a low-slope roof while not utilizing adequate fall protection system. Potential fall heights were measured to be 13'-9" with a roof pitch of 3/12.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. On April 10, 2018, employees were observed tearing off a commercial roof structure while not utilizing a fall protection system. the employer did not have a written adequate certification record to verify the employee had been trained in fall protection. Potential fall height was measured to be 13'-9" with a roof pitch of 3/12. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
