29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: On or about April 09, 2018, and at times prior thereto, the employer did not provide and require use of flame resistant clothing (FRC) when employees were working in areas where there was a potential for hydrocarbons to be present, exposing the employees to fire and explosion hazards.
