On or about April 10, 2018, at the above referenced inspection site, employees performing residential roofing operations from a sloped roof were not protected from falls greater than 6 feet, exposed to eye injuries, and exposed to head injuries. The employer failed to instruct the employees to recognize and mitigate the exposure to these hazards. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Employees where operating pneumatic nail guns, hammer staplers, and other tools. Employees were thereby exposed to eye injury from flying / falling debris. Nico Roofing & Exteriors Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1047432, citation number 1, item number 2 and was affirmed as a final order on July 16, 2015, with respect to a workplace located at 155 E. Old Bridge Palatine, IL 60067. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. On or about April 10, 2018, at the above referenced inspection site, employees performing residential roofing operations from a sloped roof were not protected from falls greater than 6 feet. The employer failed to ensure employees were utilizing fall protection while working on the roof. Employees were thereby exposed to falls of approximately 8 feet at the eve and 22 feet at the peak of the structure. Nico Roofing & Exteriors, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1149042, citation number 1, item number 1, and was affirmed as a final order on August 02, 2016, with respect to a workplace located at 19721 Greenview Lane, Mokena, Illinois 60448. Nico Roofing & Exteriors Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1047432, citation number 1, item number 2 and was affirmed as a final order on July 16, 2015, with respect to a workplace located at 155 E.
