29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about April 10, 2018 at 23831 Dablin Dr., Fairhope, Al: The employer failed to ensure employees installing sheathing on a residential house was protected from fall hazards of 8 feet 11 inches, in that they were not connected to a proper anchor and to their harness with a rope grab device.  b) On or about April 10, 2018 at 23831 Dablin Dr., Fairhope, Al: The employer failed to ensure employees installing bracing to the trusses on a residential house was protected from fall hazards of 19 feet 3 inches to over 25 feet to the concrete floor below.

29 CFR 1926.502(d)(6)(i): A snaphook not of the locking type was engaged (i) directly to webbing, rope or wire rope; (ii) to each other (iii) to a Dee-ring to which another snaphook or other connector is attached;(iv) to a horizontal lifeline; or (v) to any objectwhich is incompatibly shaped or dimensioned in relation to the snaphook such that unintentional disengagement could occur by the connected object being able to depress the snaphook keeper and release itself:  a) On or about April 10, 2018 at 23831 Dablin Dr., Fairhope, Al: The employer failed to ensure employees using personal fall arrest was properly anchored in that their rope was tied to a non-approved snap hook as part of their fall protection which was wrapped around a beam and then hooked back to the rope while installing the sheathing and bracing.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On or about April 10, 2018 at 23831 Dablin Dr., Fairhope, Al: The employer failed to ensure a proper ladder that extended three feet above the landing area was available which exposed employees to potential fall hazards while getting on and off the ladder in that the employees climbed the cross bracing of the walls to get up and down.
