29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) On or about April 11, 2018, the employer did not have a lock out program for the control of the hazardous energy, which includes machine specific procedures, training, and periodic inspections, while performing work such as, but not limited to, blade changes on the table saw and band saw and working under vehicles. Failure to lock out the energy sources while these tasks are being performed exposes employees to injuries.

Employees that use the saw to cut plywood and luan board are exposed to amputation injuries.
