29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork:     The employer is failing to protect employees from fall hazards associated with working off of a scaffold platform that is not secured to the forklift used to raise it.  The most recent occurrence of this was evident during the inspection of the job site located at 18321 Grant St. Omaha, NE where the employer has an employee working off a platform lifted with a forklift without securing the platform as to prevent it from falling off the forks.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:                                  The employer is failing to protect employees from electrical hazards associated with using damaged electrical cords to power portable hand tools.  The most recent occurrence of this was evident during the inspection of the job site located at 18321 Grant St. Omaha, NE where the employer has an employee using a Skillsaw circular saw to cut the ¾ inch plywood sheathing to fit without providing adequate strain relief resulting in the insulation pulling free from the electrical cords.      Garcia Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.405(g)(2)(iv), which was contained in OSHA inspection number 1102541, citation number 1, item number 1 and was affirmed as a final order on , with June 21, 2016 respect to a workplace located at 2023 S. 212th St. Elkhorn, NE.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection plan which meets the requirements of �1926.502(k) developed and implemented.                                                                                                          The employer is failing to protect employees from fall hazards associated with sheathing the roof of a new residential home without fall protection.  The most recent occurrence of this was evident during the inspection of the job site located at 18321 Grant St. Omaha, NE where the employer has an employee sheathing the roof without fall protection and the ground to eave is greater than 6 feet.    Garcia Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1102541, citation number 1, item number 2 and was affirmed as a final order on June 21, 2016, with respect to a workplace located at .2023 S. 212th Street, Elkhorn, NE.
