29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity.  On 1/20/2018, at the job site located at 15 Rock Rose Drive, Covington, Georgia, the employer did not ensure the scaffold employees used was inspected by a competent person prior to permitting employees to work from that scaffold. An employee fell and broke his arm when working from the scaffold.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  On 1/30/2018, the employer did not create and implement a scaffold training program so as to educate employees in the recognition and elimination of hazards identified with scaffold work.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  On 1/30/2018, at the 15 Rock Rose Drive, Covington, Georgia job site, employees were exposed to fall hazards from the second story breezeway as guardrail systems did not extend across the entire breezeway to guard the fall greater than 6 ft. fall to lower levels and was missing the lower guardrail.    No abatement certification or documentation is required for this item.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  On 1/30/18, the employer did not develop and implement a training program for employees to enable them to identify and eliminate fall hazards they were exposed to on residential job sites.
