29 CFR 1926.651(b)(1): The estimated location of utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, was not determined prior to opening an excavation.  a) On or about March 28, 2018, Isaiah Apartments Construction Site Lot 105:  Highmark Excavation L.L.C. was digging an approximate 3 foot deep excavation on a residential construction site in the presence of underground utilities that included a 3/4 inch gas line at a depth of approximately 1 1/2 feet.   The gas line was not located with a current utility locate ticket prior to the excavation work on March 28, 2018, and was severed during site operations.    The gas line was identified by a utility locate service with locate ticket #2018030302 on January 17, 2018, but the locate paint for the gas line was not present on March 28, 2018, and this ticket had expired in 21 days after it was conducted.  Note:   Abatement certification is required for this item.
