29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) Rebar Building: On or about April 11, 2017, employees were exposed to amputation and crushing hazards by an unguarded point of operation on the rebar bending machine, serial number BMC 161457. b) Rebar Building: On or about April 11, 2017, employees were exposed to amputation and crushing hazards by an unguarded point of operation on the rebar bending machine, stock number 17374. c) Upper Fabrication Area: On or about April 11, 2017, employees were exposed to amputation and crushing hazards by an unguarded point of operation on the Piranha P50 Ironworker. d) Fabrication Area: On or about April 11, 2017, employees were exposed to amputation and crushing hazards by an unguarded point of operation on the Cincinnati Press Brake, serial number 54377. Anderson Steel, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(3)(ii), which was contained in OSHA inspection number 954316, citation number 1, item number 1/b and was affirmed as a final order on 04/04/14, with respect to a workplace located at 3811 River Drive N in Great Falls, Montana.
