29 CFR 1926.28(a): The employer does not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions or where this part indicates the need for using such equipment to reduce the hazards to the employees:  a) The Employer does not require and ensure the use of personal protective equipment.  This violation was most recently observed on April 11, 2018, at a worksite on 3705 Riva Ridge in Boise, ID, where employees used nail guns without approved safety glasses while performing re-roofing activities.  To abate this violation, the employer must ensure that its employees are protected from eye injuries by providing approved safety eyewear and taking steps to ensure use.  Note) Abatement certification AND supporting documentation are required for this item.  Julio Alvarado dba Alvarado Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.28(a), which was contained in OSHA inspection number 991528, citation number 1, item number 1 and was affirmed as a final order on September 1, 2014, with respect to a workplace located at 4313/4315 Vera, Boise, Idaho 83705.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels are not protected by guardrail systems, safety net system, or personal fall arrest system:  a) The Employer does not require the use of any form off all protection during roofing work. This violation was most recently observed on April 11, 2018, at a worksite on 3705 Riva Ridge in Boise, ID, where employees performing re-roofing activities were not protected from fall hazards up to 17 feet above the ground.  To abate this violation, the Employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof more than 6 feet high without adequate fall protection.  Note) Abatement certification AND documentation are required for this item. 29 CFR 1926.503(a)(2)(iii): The employer does not assure that each employee had been trained, as necessary, by a competent person qualified in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used:    a) The Employer does not adequately train employees in identifying fall hazards and the use of fall protection.  This violation was most recently observed on April 11, 2018, at a worksite at 3705 Riva Ridge in Boise, ID, where employees did not recognize or acknowledge the hazard of performing roofing activities on a 6 in 12 pitched roof without a fall protection system in place.  To abate this violation, the Employer must provide training to all workers before they are exposed to fall hazards.  The training must teach each employee how to recognize the hazards of falling and how to minimize these hazards, including specific procedures for installation and use of fall protection systems.  The Employer must also verify training by preparing and maintaining a written certification record.  Note) Abatement certification AND supporting documentation are required for this item.
