29 CFR 1926.652(d)(2): Manufactured materials and equipment used for protective systems were not used and maintained in a manner that was consistent with the recommendations of the manufacturer: On or about April 12, 2018 an employee was working inside of a Modular Aluminum Panel Shield Box that was installed without a spreader on one side, exposing the employee to the hazards associated with a shift in the trench shield. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
