29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition: On or about April 12, 2018, the saw department had a variety of steel bar stock on the floor, on stands and on racks. Employees had to step on, walk on and stand on the steel bars while using the overhead crane to retrieve stock for the saw. There were no walking working aisles established.

29 CFR 1910.179(j)(2): Frequent inspections of the cranes were not performed at intervals as generally defined in paragraph (j)(1)(ii)(a) of this section or as specifically indicated: On or about April 12, 2018, a frequent inspection of the cranes was not performed. The overhead cranes such as but not limited to the 5 ton Wright overhead crane in the saw department was missing bridge bumpers. The Electrolift hoist at the CNC machine had a pendant controller with the electrical wiring insulation damaged. The 10 ton Cleveland overhead crane in the production area was not marked with the load capacity and the Shaw box crane in the center of the production building that was between two cranes on the same rail system had a broken gear box that would not allow the crane to travel the rails on its own power.

Employees use the overhead cranes throughout the departments to move large steel stock and large parts from storage to machinery. The employer was using a monthly cable, drum and hook inspection sheet as their annual inspection of their cranes.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about April 12, 2018, the Johnford CNC ST-60CH lathe in the production area had a non-working interlock at the top of the machine door opening. While the machine is running with the door open and rotating the part, an employee uses a hand file to remove any burrs on the edge of the rotating part.

There was a 110/220 volt electrical distribution panel with an open cover. Two of the over current devices were missing from the panel and were not blanked.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings were not protected from abrasions: On or about April 12, 2017, there were two industrial pedestal fans with control switch wiring that was subject to abrasion. The electrical junction boxes were damaged and not securely attached to the back side of the fan housings. One of the fans were in the CNC room and the other fan was in the production area.
