On or about April 12, 2018, located at the Fieldstone Trace Clubhouse project, employees were installing shingles on a steep-pitch roof that was approximately a 12 on 12 pitch without adequate fall protection and not wearing eye protection while operating a pneumatic nail gun. The employer never initiated or maintained safety and health programs to protect employees from the hazards associated with performing roofing work on a steep-pitch roof.

On or about April 12, 2018, located at the Fieldstone Trace Clubhouse project, employees were installing shingles on a steep-pitch roof that was approximately a 12 on 12 pitch without adequate fall protection and not wearing eye protection while operating a pneumatic nail gun. The employer never instructed employees in the recognition and avoidance of unsafe conditions with the regulations applicable to installing shingles while on a steep-pitched roof in order to protect the employees from associated safety and health hazards.
