The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. a. On or about April 11, 2018, at the above addressed jobsite, employees conducting residential roofing operations were not wearing PFAS with the attachment point at the employee's back approximately two (2) stories, thereby exposing the employees to the hazards associated with falls.
