29 CFR 1926.451(e)(1): When scaffold platforms were more than 2-feet (0.6m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral pre-fabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface were not used: (a) On or about April 10, 2018, employees doing masonry work at a single family residential home located at 5812 Folsum Place, McKinney, TX: Employees working from a two-tier scaffold systems washing bricks did not have a safe means of access and were exposed to fall hazards of approximately 14-feet to the lower level.
