29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels: On or about April 17, 2018 at the above addressed job site, an employee performing manufacturing of wood doors and forms using a molding machine was exposed to noise dose of 69.11% or an equivalent of 87.3 dBA which was capable of causing permanent hearing loss. Monitoring was performed for 454 minute sampling time and zero exposure for the remaining 26 minutes of the 8 hour shift.
